New Patients must carry with them the following if not previously faxed from your Doctors office. 1. A referral from your Primary Care Physician (PCP) or Surgeon. 2. Relevant Dr's notes (Soap Notes) and or visit summaries, at least the last 3 visits. 3. MRI Paper Reports done within the last 3 years. All Records will be verified via fax from your referring Physician prior to admittance. Cost of Preliminary appointments $150 Credit Debit or Money Orders Only.
